肝脂酶基因启动子-514C/T多态性与糖尿病慢性肾脏疾病的相关性研究

  【摘要】 目的 探讨肝脂酶(HL)基因启动子区-514C/T多态性与糖尿病慢性肾脏疾病(CKD)的相关性。 方法 纳入T2DM患者(T2DM组,n=164),根据尿白蛋白(UA1b)再分为CKD亚组(n=108)和无CKD亚组(CKD0,n=56);另选健康对照者(NC组,n=91)。运用聚合酶链反应——限制性片段长度多态性(PCR-RFLP)技术,对各组HL基因启动子区-514C/T多态性进行检测,并比较各组间基因型频率和等位基因频率及相关资料。 结果 (1)CKD组CT+TT基因型和T等位基因频率高于CKD0组(P<0.05);(2)NC组与T2DM组间基因型和等位基因频率差异无统计学意义(P>0.05);(3)T2DM组中C/T+T/T基因型患者TG水平高于C/C基因型者(P<0.05);(4)Logistic回归分析表明,HL基因启动子区-514C/T基因多态性与CKD显著相关。 结论 HL基因启动子区-514T等位基因可能是CKD发生的遗传易感因素。

  【Abstract]】 Objective To investigate the association of between hepatic lipase(HL) gene promoter region 514C/T polymorphism with and diabetic nephropathy chronic kidney disease (CKD) in diabetes(DN). Methods The polymorphism of HL gene promoter 514C/T in 164 patients with type 2 diabetesT2DM included 56 patients with pure T2DM (CKD0,n =56) group,DN0 group, 108T2DM patients with diabetic nephropathy CKD(DNCKD group,n=108)group] and 91 normal controls(NC group) were detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). The genotype frequency and allele frequency and relative clinical data were compared between among groups. Results (1) The C/T + T/T genotype frequencies and T allele frequencies in DN group were significantly higher in CKD group higher than in DNCKD0 group(P<0.05).; (2)There were no significant differences in genotype frequencies and allele frequencies between NC and T2DM(DNCKD0+DNCKD) group (P>0.05).; (3)In T2DM group, triglyceride(TG) was significantly higher in C/T + T/T genotype group than in C/C genotype group(P<0.05).;(4) Logistic regression analysis showed that HL gene promoter region 514C/T polymorphism were significantly related to DNCKD. Conclusions The HL-514T allele may be the genetic predisposing factor for DNCKD development.
