  2型糖尿病巨噬细胞抑制因子-1水平的改变及其临床意义

  【摘要】目的 探讨胰升血糖素样肽-1(GLP-1)水平与T2DM和体重的相关性及临床意义。方法 选取93例新诊断T2DM患者按BMI分为超重亚组(A组,46例)和正常体重亚组(B组,47例),选取NGT者作为对照组(NC组,43例)。测量研究对象身高、体重、WC、血糖、血脂、胰岛素等指标,计算BMI,胰岛素抵抗指数(HOMA-IR);ELISA法测定血清GLP-1水平。结果 A组血清GLP-1浓度低于B、NC组(P<0.05)。简单相关分析显示,GLP-1与BMI(r=-0.647,P<0.01)、WC(r=-0.401,P<0.01)、TC(r=-0.287,P<0.05)呈负相关,与HDL-C呈正相关HDL(r=0.284,P<0.05)。多元线性逐步回归分析显示,BMI(β=-0.458,P<0.01)、TC (β=-0.416,P<0.01)为血清GLP-1水平的独立影响因素。结论 T2DM患者GLP-1水平降低,伴有超重患者降低更明显,其与HDL呈正相关,与BMI、腰围、TC呈负相关,BMI、TC是GLP-1水平的独立相关因子。

  【Abstract】 Objective To evaluate the association of serum level of glucagon-like peptide 1 (GLP-1) with T2DM and the patients’ weight. Methods Ninety-three patients with newly diagnosed T2DM were divided into overweight group (group A) and normal weight group (group B) according to BMI ≥ or <25kg/m2, while 43 subjects with NGT were put into the control group. Height, weight, waist circumference (WC), fasting plasma glucose, FIns, lipids, and so on were determined in all subjects for calculating BMI and HOMA-IR. The level of serum GLP-1 was measured by enzyme-linked immunosorbent assay (ELISA). Results The level of serum GLP-1 in group A was lower than that in group B and NC group (P<0.05). Simple correlation analysis showed that the level of GLP-1 was negatively correlated with BMI (r=-0.647, P<0.01), WC (r=-0.401, P<0.01), and TC (r=-0.287, P<0.05), while positively with HDL (r=0.284, P<0.05). Furthermore, multivariate linear regression analysis showed that BMI (β=-0.458, P<0.01) and TC (β=-0.416, P<0.01) were the independent factors of serum level of GLP-1. Conclusion The level of GLP-1 in T2DM patients was lower, and in the overweight T2DM patients its decrease was more significant. The level of GLP-1 is positively correlated with HDL, while negatively correlated with BMI, WC, and TC. BMI and TC are the independent influential factors of the serum level of GLP-1.
