二甲双胍对2型糖尿病患者血清Vaspin水平影响的研究

  【摘要】 目的 探讨二甲双胍对T2DM患者血清内脏脂源性丝氨酸蛋白酶抑制剂(vaspin)水平的影响。 方法 选择109例T2DM患者随机分为二甲双胍治疗(A)组、格列美脲治疗(B)组、饮食运动治疗(C)组,30名健康体检者为对照(D)组。。采用ELISA法测定各组vaspin水平,分析血浆vaspin水平与血脂、FPG、2 hPG、FIns、BMI、HOMA-IR等的关系。观察A、B、C组治疗前后vaspin水平变化。 结果 (1)A组vaspin水平高于D组;vaspin水平女性高于男性;(2)vaspin水平与性别、BMI、WHR、HOMA-IR、FIns呈正相关(P<0.05);与HDL-C呈负相关(P<0.05);(3)A组治疗14周后vaspin水平降低;B组水平无明显变化;C组降低,差异均有统计学意义(P均<0.05);(4)HOMA-IR与BMI是影响vaspin水平的独立相关因素。 结论 二甲双胍增加T2DM患者IS同时,降低vaspin水平。

Influence of metformin on the level of serum vaspin in T2DM patients LIU Yun-tao, HU Bin, JIAN Lei, et al. Department of Endocrinology, Renhe Hospital, China Three Gorges University, Yichang Hubei 443000, China

  【Abstract】 Objective To investigate the effect of metformin on the level of plasma vaspin in T2DM patients. Methods Thirty normal subjects were selected as the controls (group A). The 109 patients with T2DM were randomized into three groups: metformin treated group (group B), glimepiride treated group (group C), diet and exercise treated group (group D). The level of plasma vaspin was measured in patients with T2DM and normal subjects by ELISA. The relationship of the level of plasma vaspin with blood lipid, HbA1c, FPG, 2hPG, BMI, WHR, and HOMA-IR was also analyzed after treatment, and the change in the level of plasma vaspin was observed in the three groups. Results (1) Group A had a higher vaspin level than the control group (0.78±0.32 vs 0.49±0.21, P<0.01). Vaspin level was higher in female than in male (0.74±0.18vs 0.47±0.15, P<0.05). (2) The vaspin level was positively correlated with sex, BMI, WHR, HOMA-IR, and FIns (r=0.532, 0.602, 0.523, 0.479, 0.530, P<0.05), and negatively correlated with HDL-C (r=- 0.598, P<0.05). (3) After the treatment for 14 weeks, the vaspin level in group A was decreased, it was not significantly changed in group B, while it was decreased in group C, and the difference was significant in statistics (P<0.05). (4) HOMA-lR and BMl were independently related factors influencing the level of serum vaspin. Conclusion Metformin can improve insulin resistance as well as reduce the level of plasma vaspin.
