  【摘要】 目的 探讨他扎罗汀诱导基因2(chemerin)在冠心病患者心外膜脂肪组织(EAT)中的表达及与冠状动脉脉粥样硬化的关系。 方法 选取53例行选择性心脏外科手术的患者,根据冠状动脉造影结果分为冠心病组(CHD,n=37)和非冠心病组(NCHD,n=16),收集临床资料,进行Gensini冠状动脉评分;体外循环前分别取两组患者的EAT和皮下脂肪组织(SAT),采用RT-PCR法检测脂肪组织中chemerin、ChemR23、脂联素(APN)与TNF-αmRNA的表达。 结果 与NCHD组相比,CHD组EAT中chemerin、TNF-α表达水平升高,APN表达减低(P均<0.01),但两组间ChemR23表达差异均无统计学意义(P均>0.05);CHD组chemerin在EAT中表达高于SAT(P<0.05),NCHD组chemerin表达差异无统计学意义(P>0.05);EAT中chemerin基因表达与Gensini冠状动脉评分、BMI、腰围、FPG呈正相关,与APN表达呈负相关。 结论 冠心病患者EAT中chemerin表达升高,冠状动脉脉粥样硬化越严重,chemerin基因表达水平越高。

  【Abstract】 Objective To explore the chemerin expression in epicardial adipose tissue (EAT) in patients with coronary artery disease (CAD) and its association with the severity of CAD. Methods Fifty-three patients undergoing selective heart surgery were chosen and divided into CAD group (n=37) and non-CAD group (n=16) according to the result of coronary angiography, then the clinical data were collected and the coronary artery Gensini scoring was carried out. The EAT and thoracic subcutaneous adipose tissue (SAT) were obtained before cardiopulmonary bypass. And the expressions of chemerin, chemR23, adiponectin (APN), and TNF-а in adipose tissue were detected by RT-PCR. Results Comparing with the non-CAD group, the expressions of chemerin (P<0.01) and TNF-а (P<0.01) of the CHD group were significantly higher, the adiponectin expression was significantly lower (P<0.01), but there was no difference in the chemR23 expression in EAT between two groups (P>0.05). In the CAD group, the chemerin expression in EAT was significantly higher than in SAT (P<0.05), however, such difference did not exist in the non-CAD group (P>0.05). The chemerin gene expression in EAT was positively correlated with Gensini score (r=0.365, P<0.05), BMI (r=0.305, P<0.05), waist circumference (r=0.384, P<0.01), and fasting blood glucose (r=0.334, P<0.05), while negatively correlated with adiponectin expression (r=-0.322, P<0.05). Conclusions Chemerin expression in EAT of patients with CAD is significantly elevated, and the more serious coronary atherosclerosis is, the higher chemerin gene expression level is.
