二甲双胍和/或格列吡嗪治疗对2型糖尿病患者血清内脏脂肪素水平及胰岛β细胞功能影响的研究

  【摘要】 目的 研究二甲双胍格列吡嗪复方制剂及单药二甲双胍或格列吡嗪治疗对T2DM患者血清内脏脂肪素(VF)水平及胰岛β细胞功能的影响。方法 测定治疗前后空腹及餐后血糖、血清胰岛素、HbA1c、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、VF等相关指标。结果 治疗后二甲双胍格列吡嗪复方制剂组及格列吡嗪组血清VF水平较治疗前下降(P<0.01);治疗后二甲双胍格列吡嗪复方制剂组HOMA-β增加(P<0.01);Pearson相关性分析显示,治疗后VF水平与FPG、2 hPG及HbA1c呈正相关(r=0.227,P<0.05;r=0.201,P<0.05;r=0.199,P<0.05);与2 hIns呈负相关(r=-0.105,P<0.05)。结论 二甲双胍格列吡嗪复方制剂在有效控制血糖的同时,还能降低VF水平并改善胰岛β细胞功能。VF水平的变化与血糖及胰岛素水平相关,可能在T2DM发生发展及临床治疗中发挥作用。

  【Abstract】 Objective To evaluate the effect of compound metformin-glipizde and single metformin or glipizde on the serum visfatin levels and β cell functions of the T2DM patients. Methods The levels of fasting and postprandial blood glucose, insulin, glycosylated HbA1c, HOMA-IR of insulin resistance, and of HOMA-β and serum visfatin were investigated before and after the treatment. Results After treatment, in the compound metformin-glipizde and single glipizde groups there was a significant reduction of visfatin level (P<0.01), and in the compound metformin-glipizde group there was an increase of HOMA-β (P<0.01). The Pearson analysis showed that the serum visfatin level was positively correlated with FPG, 2 hPG and HbA1c (r=0.227, P<0.05; r=0.201, P<0.05; r=0.199, P<0.05), and negatively correlated with 2 hIns (r=-0.105, P<0.05) after treatment. Conclusion The compound metformin-glipizde is effective in reducing the serum visfatin and improving the β cell functions at the same time of controlling blood glucose. The change of level of serum visfatin is related with the blood glucose and insulin levels, contributing to the novel strategy for the treatment of T2DM.
