高敏C反应蛋白对无高血压病史糖尿病人群新发心脑血管事件的影响

  【摘要】 目的 探讨基线高敏C反应蛋白(hsC-RP)对无高血压病史的DM人群新发心脑血管事件的影响。 方法 采用前瞻性队列研究方法,以参加健康体检的101510名开滦集团职工中FBG≥7.0mmol/L或已确诊为DM,正在使用降糖药物的2973例DM患者作为观察队列,平均随访(47.26±5.38)个月。 结果 基线hsC-RP水平增高,心脑血管事件发病率、心脑血管事件死亡率、全因事件死亡率增高(P<0.05,Log-rank检验)。Cox风险模型表明,hsC-RP>3.0mg/L者发生心脑血管事件、心脑血管死亡事件、全因死亡事件的风险分别是hsC-RP≤3.0mg/L者的1.63倍(95%CI:1.03~2.57)、2.62倍(95%CI:1.12~6.12)和1.73倍(95%CI:1.06~2.84)。 结论 基线hsC-RP>3.0mg/L增加无高血压病史的DM人群新发心脑血管事件的风险。

  【Abstract】Objective To evaluate the predictive value of baseline serum high sensitivity C-reactive protein (hs C-RP) for the first cardio-cerebral vascular event in the non-hypertensive diabetic population. Methods In this prospective cohort study, a total of 101510 employees of Kailuan Group, who received healthy examination from 2006 to 2007, were screened and 2973 subjects with fasting plasma glucose ≥7.0mmol/L or known diabetes mellitus and under insulin or hypoglycemic drug therapy and free from hypertension were followed up for (47.26±5.38) months. Results Incidence of total cardio-cerebral vascular events, cardio-cerebral vascular mortality and all-cause mortality increased in the patients with increased level of baseline hs C-RP (P<0.05, Log-Rank test). Cox proportional hazard model showed that the individuals with hs C-RP >3 mg/L had higher risk for cardio-cerebral vascular events (RR: 1.63, 95%CI: 1.03~2.57), cardio-cerebral vascular mortality (RR: 2.62, 95%CI: 1.12~6.12), all-cause mortality (RR:1.73, 95%CI: 1.06~2.84) compared with whose hs C-RP ≤3mg/L. Conclusion The baseline hs C-RP >3.0mg/L increases the risk of the first cardio-cerebral vascular event in the diabetic population without history of hypertension.
