糖调节受损和2型糖尿病患者血浆骨连接素水平改变

【摘要】目的探讨NGT、IGR和T2DM患者血浆骨连接素(SPARC)水平的变化及其与BMI、WHR、胰岛素抵抗指数(HOMA-IR)、FIns、血脂等的关系。方法采用酶联免疫法测定了54例T2DM患者、53名IGR人群和53例NGT患者的血浆SPARC水平,并分析了血浆SPARC水平与BMI、WHR、FIns、HOMA-IR等的关系。结果T2DM患者血浆SPARC水平明显高于IGR患者和NGT人群[(16.74±6.99 vs 14.04±8.03或11.72±4.47) μg/L, P<0.05和P<0.01]。相关分析显示,血浆SPARC水平与BMI、体内脂肪百分比(Fat%)、TG、FIns、2 hIns、HOMA-IR呈明显正相关(r=0.19, r=0.20, r=0.22, r=0.21, r=0.16, r=0.27, P<0.05或P<0.01),多元回归分析结果表明,TG和HOMA-IR是血浆SPARC水平的独立影响因素。结论血浆SPARC水平的改变与糖代谢紊乱和IR有关,并可能参与了IR和T2DM的发生和发展。

【Abstract】ObjectiveTo investigate the changes of plasma SPARC levels and the relationship of plasma SPARC levels with HOMA-IR, body mass index (BMI), waist hip ratio (WHR), fasting insulin (FIns) and other factors in patients with impaired glucose regulation (IGR) and type 2 diabetes (T2DM).MethodsPlasma SPARC level was assayed by ELISA in 54 patients with T2DM, 53 patients with IGR and 53 normal controls. The relationship of plasma SPARC levels with BMI, WHR, HOMA-IR, and FIns was also analyzed in these subjects.ResultsPlasma SPARC levels in patients with T2DM were significantly elevated as compared with those in IGR patients and normal controls [(16.74±6.99 vs 14.04±8.03 and 11.72±4.47) μg/L, P<0.05 and P<0.01]. Plasma SPARC levels were positively correlated with BMI, FAT%, TG, FIns, 2 hIns, and HOMA-IR (r=0.19, r=0.20, r=0.22, r=0.21, r=0.16, r=0.27, P<0.05 and P<0.01). Multiple regression analysis showed that TG and HOMA-IR were the independent related factors influencing plasma SPARC levels.ConclusionThe changes of SPARC levels are associated with glucose metabolism disorder and insulin resistance, and it may contribute, at least in part, to the pathogenesis of IR and T2DM.
