【摘要】 目的 探讨T2DM患者微量尿白蛋白(U-mAlb)、尿白蛋白/尿肌酐(UACR)与振动感觉阈值(VPT)及其影响因素的相关性。 方法 选取T2DM患者540例,根据UACR分为正常尿白蛋白(NAU)组、微量尿白蛋白(MAU)组和大量尿白蛋白(OAU)组。测定各组VPT、HbA1c等指标。 结果 MAU组和OAU组糖尿病病程、HbA1c和VP高于NAU组(P=0.000、0.008、0.000)。UACR与VPT、糖尿病病程、HbA1c呈正相关(P=0.002、0.003、0.004)。多元逐步回归分析显示,糖尿病病程和HbA1c是UACR的独立危险因素(P=0.015、0.011)。 结论 糖尿病病程是UACR的独立危险因素,随蛋白尿程度加重,VPT异常的发生率增加。

Relationship between albuminuria and vibration perception threshold in patients with type 2 diabetes mellitus
【Abstract】 Objective To investigate the relationship between urinary albumin creatinine ratio(UACR) and vibration perception threshold(VPT) in patients with type 2 diabetes mellitus (T2DM). Methods VPT, UACR, glycolated hemoglobin(HbA1c) were tested in 540 T2DM cases. They were assigned to normal albuminuria group (NAU), micro-albuminuria group(MAU) and overt-albuminuria group(OAU) according to the value of UACR. Results Compared with NAU group, the duration of diabetes, HbA1c and VP is significantly higher in MAU group and OAU group(P=0.000, 0.008, 0.000). VPT, diabetes duration and HbA1C were positively correlated with UACR(P=0.002, 0.003, 0.004). Multiple stepwise regression analysis showed that diabetes duration (r=0.072, P=0.015) and HbA1c(r=0.054, P=0.011)are independent risk factors of UACR. Conclusion Diabetic duration is an independent risk factor of UACR. With the degree of proteinuria increased, the abnormality of VPT get worse.
