 新诊断2型糖尿病患者与健康成人心率变异性的比较

  【摘要】 目的 比较新诊断T2DM患者与健康成人的心率变异性(HRV)。 方法 选取新诊断T2DM患者(T2DM组)54例和健康成人(NC)组50名,应用12通道动态心电图检测两组HRV时域和频域指标,并进行对比分析。 结果 T2DM组24 h全部窦性RR间期的标准差(SDNN)、相邻RR间期差值均方根(rMSSD)、总功率(TP)、低频功率(LF)和高频功率(HF)均低于NC组[(116.14±40.31) vs (130.52±39.60) ms、(28.10±15.20) vs (33.14±12.17) ms、(2391.60±2341.30) vs (3120.93±1890.61) ms2、(487.30±561.80) vs (678.42±592.65) ms2、(203.62±275.60) vs (295.67±258.12) ms2,P<0.05],但24 h连续5分钟RR间期均值标准差(SDANN)、极低频功率(VLF)与NC组比较,差异无统计学意义[(122.36±43.91) vs (114.51±40.17) ms、(2305.48±1451.26) vs (2265.91±1321.17) ms2,P>0.05]。 结论 新诊断T2DM患者心脏自主神经调控功能紊乱,迷走神经活性降低,交感神经活性相对增强。

  【Abstract】 Objective To compare heart rate variability (HRV) between newly diagnosed type 2 diabetic patients and healthy people. Methods 54 cases with newly diagnosed type 2 diabetic patients were seleceted as T2DM group and 50 healthy people as normal control (NC) group. The HRV time domain and frequency domain indicators were calculated in all subjects by 24 h dynamic electrocardiogram record, and the results were analyzed statistically. Results All 24 h period between sinus rr standard deviation (SDNN),phase difference between adjacent rr RMS (rMSSD),total power (TP),Low Frequency component (LF) and high frequency component (HF) were lower in T2DM group than in NC group [(116.14±40.31) vs (130.52±39.60) ms、(28.10±15.20) vs (33.14±12.17) ms、(2391.60±2341.30) vs (3120.93±1890.61) ms2、(487.30±561.80) vs (678.42±592.65) ms2、(203.62±275.60) vs (295.67±258.12) ms2,all P<0.05],but there were no significant difference in 24 h period between rr mean standard deviation sdnn for five minutes (SDANN )and Very Low Frequency component (VLF) between the two groups [(122.36±43.91) vs (114.51±40.17) ms、(2305.48±1451.26) vs (2265.91±1321.17) ms2,P>0.05]. Conclusions The activity of vagus nerve attenuates, while the activity of sympathetic nerve accentuates relatively in newly diagnosed type 2 diabetic patients.
