【摘要】本文讨论了基础胰岛素类似物(地特胰岛素)在特殊糖尿病人群中的应用。对于T1DM的儿童和青少年，其使用灵活、夜间低血糖风险小，餐后低血糖风险小，提高了孩子及其家庭的生活质量。妊娠糖尿病优化血糖管理有助于改善母亲和新生儿的预后，其能提供良好的血糖控制且在孕妇中耐受良好。对于老年糖尿病患者，低血糖与心梗、死亡风险、痴呆及骨折等不良事件相关，其可提供有效的血糖控制且对老年患者低血糖风险小。地特胰岛素由于其作用特点，满足了特殊糖尿病患者的治疗需求。

　　【Abstract】This review aimed to assess the application of basal insulin analogs(insulin detemir, Det)in the diabetic specific populations. All children with type 1 diabetes mellitus require insulin therapy. Det can offer flexibility, reduction of nocturnal hypoglycemic episodes and decrease in postprandial hyperglycemic events, resulting in improved quality of life for diabetic children and their families. Excellent glycemic control is essential in pregnancy to optimize maternal and fetal outcomes. IDet can provide good glycemic control and be well tolerated in gestational diabetes. In elderly patient a history of hypoglycemia is correlated with myocardial infarction, mortality, dementia and fracture. Treatment with Det can provide effective glycemic control without serious hypoglycemia in elderly patients. Basal insulin detemir has characteristics that may meet the treatment needs of diabetic specific populations.
