　　【摘要】 报道1例以T1DM起病的IgG4相关性疾病(IgG4-RD)患者，逐渐出现胰腺、肾脏、垂体等多系统受累症状。分析病例资料并复习相关文献，提示IgG4-RD可以早期出现内分泌相关症状，甚至为首发症状。患者易被误诊、漏诊，临床医生需提高警惕。

　　【Summary】 A patient has type 1 diabetes mellitus as first manifestation of IgG4-related diseases(IgG4-RD), and gradually develops multi-system symptoms including pancreatitis, kidney injury and hypophysitis. Endocrine system symptoms may present early, even as the first manifestation in IgG4-RD. Patients with this disease can be misdiagnosed or the correct diagnosis can be missed. Clinicians need to be aware of this disease.
