[Abstract] Objective To compare the MMP-1 and gingival crevicular fluid (GCF) levels in GCF of patients with type 2 diabetes mellitus, periodontal disease, and healthy persons, and to investigate the relationship between MMP-1 concentration in GCF of T2DM patients and glycometabolic condition. Methods GCF collection was performed with the filter paper method. Thirty patients of T2DM group and T2DM merging periodontal diseases group, and thirty healthy controls were selected in this study. The MMP-1 levels in GCF were measured by ELISA, and HbA1c levels were measured by high-performance liquid chromatography. Results The diabetic group and T2DM group with periodontal diseases had higher GCF as compared to the normal control group (P<0.01). The MMP-1 level in GCF of T2DM group with periodontal diseases was higher than that in the diabetic group and the normal control group [(5.02±0.16) vs (3.01±0.09), (2.24±0.11) μl/sample] (P<0.01). The MMP-1 level in GCF of diabetic group and normal control group had no significant difference (P>0.01). The MMP-1 level in GCF of T2DM group with periodontal diseases was not correlated with HbA1c levels. Conclusion Among three groups, GCF and MMP-1 levels are the highest in T2DM group with periodontal diseases and the lowest in normal control group, which suggested that periodontitis is the main reason affecting the results. MMP-1 level is lower in diabetic group than that in T2DM group with periodontal diseases, which suggested that the periodontal disease susceptibility in diabetics is not just caused by changes of MMP-1 level, but might be the results of several factors working together.
