  无症状性高尿酸血症与代谢综合征及其组分相关性的研究

  【摘要】 目的 调查无症状性高尿酸血症(HUA)的患病特点,探讨血尿酸(SUA)水平与代谢综合征(MS)及其组分的相关性。 方法 通过随机整群抽样,对山东铝业公司职工301人进行调查与体检,并进行血液检查。 结果 HUA总检出率为22.3%,男性高于女性(P<0.01);MS检出率为25.2%,其组分超重或肥胖、高血糖、高血压、血脂异常的检出率分别为59.8%、18.9%、36.2%、34.9,而HUA组患者MS及超重或肥胖、高血糖、高血压、血脂异常的检出率均高于NUA组(P<0.05);HUA组中具有2个及以上MS组分数目的患者明显多于NUA组(P<0.01);SUA水平与BMI、WC、SBP、TC、TG、LDL-C呈正相关,与HDL-C呈负相关;WC(OR=1.111,95%CI 1.053~1.172)、TG(OR=1.288,95%CI 1.015~1.634)、LDL-C(OR=1.739,95%CI 1.200~2.519)是发生HUA的相关因素。 结论 SUA水平与MS及其组分密切相关,其中WC、TG与LDL-C是发生HUA的相关因素。

  【Abstract】Objective To investigate the characteristics of asymptomatic hyperuricemia (HUA), and evaluate the association of serum uric acid (SUA) level and metabolic syndrome (MS) and its components. Methods A random cluster sampling was conducted with 301 employees of Shandong Aluminum Company. The subjects had physical and serological examinations. Hyperuricemia was defined as ≥417umoL/L (in men) and ≥357umol/L (in women). MS was defined using the recommendation of CDS in 2004. Available data was analyzed using SPSS18.0 statistical software, and the difference was statistically significant when P<0.05. Results Total detection rate of HUA was 22.3%, and males were significantly higher than women (P<0.01). The detection rate of MS was 25.2%, and the detection rate of overweight or obese, hyperglycemia, hypertension, and dyslipidemia in HUA patients was 59.8%, 18.9%, 36.2%, and 34.9% respectively, all higher than that in the NUA group (all P<0.05). The patients having two or more components of MS in HUA group were significantly more than those in NUA group (P<0.01). Serum uric acid level was positively correlated with BMI, WC, SBP, TC, TG, and LDL-C, and negatively correlated with HDL-C. The relevant factors of HUA were WC (OR=1.111, 95%CI: 1.053-1.172), TG (OR=1.288, 95%CI: 1.015-1.634), and LDL-C (OR=1.739, 95%CI: 1.200-2.519). Conclusion Increased serum uric acid level is associated with an increased prevalence of metabolic syndrome and its components, and WC, TG, and LDL-C are the relevant factors of HUA.
