【摘要】 目的 探讨T2DM合并高尿酸血症者与IR、脂代谢紊乱、高血压的相关性。 方法 纳入866例T2DM患者,按照UA水平分为高尿酸血症(A)组和UA正常(B)组,比较两组肾功能、血脂和HOMA-IR。 结果 T2DM患者中合并高尿酸血症的患病率为15.5%,A组BMI、DBP、TG、TC、极低密度脂蛋白胆固醇(VLDL-C)、Cr、HOMA-IR、尿微量白蛋白/肌酐(ACr)、动脉硬化指数(ASI)均高于B组,eGFR、ISI低于B组(P<0.05)。相关性分析显示肾功能正常者中UA与血脂、HOMA-IR和肾功能存在相关性。 结论 T2DM患者高尿酸血症与血脂异常、IR、肥胖及高血压相关。

【Abstract】Objective To study the relationship of hyperuricemia with insulin resistance, dyslipidemia, and hypertension in T2DM patients. Methods The 866 T2DM patients were divided into the high uric acid group (serum uric acid ≥ 420umol/L in male and ≥ 360umol/L in female) and the normal uric acid group. Renal function, lipids, and insulin resistance phenotypes were compared in the 2 groups. Results The prevalence of hyperuricemia was 15.5% in T2DM subjects. Body mass index (BMI), systolic blood pressure (SBP), triglyceride (TG), total cholesterol (TC), very low-density lipoprotein cholesterol (VLDL-C), serum creatinine (Scr), insulin resistance index (HOMA-IR), albumin to creatinine ratio, and atherosclerosis index were all higher in the hyperuricemia group, while the glomerular filtration rate (eGFR) and insulin sensitivity index (ISI) were higher in the normal uric acid group (P<0.05). The result showed that the uric acid was correlated with lipids, insulin resistance, and renal function phenotypes in normal renal function individuals. Conclusion Hyperuricemia is correlated with dyslipidemia, insulin resistance, obesity, and hypertension in T2DM patients. More studies are needed to decipher plausible impacts of the elevated plasma uric acid level on the pathogenesis of atherosclerosis and/or hypertension.
