　　【摘要】 基础胰岛素是糖尿病患者控制血糖的有力武器，然而医生及患者在胰岛素起始及剂量优化过程中的顾虑限制了其作用的发挥。地特胰岛素是一种具有良好降糖效果的基础胰岛素，研究显示其在个体内变异性低、低血糖风险较低，并可减少体重增加,这些特征增加了地特胰岛素剂量优化的可行性，对糖尿病患者的血糖管理具有重要意义。

　　【Summary】 Basal insulin is a powerful weapon for controlling blood glucose in diabetes patients. However,the psychological resistance of physicians and patients to the initiation and dose optimization of insulin hinders its fuction display thoroughly.Insulin detemir is a kind of basal insulin with excellent efficacy in decreasing blood glucose.The study shows that it has a lower levels of intra-individual glucose variability,hypoglycaemic risk and body weight increase , and it increases the feasibility of dose optimization,and shows significant values to the glucose management in patients with diabetes.
