2型糖尿病患者血清血管生成素2的变化及其临床意义

  【摘要】 目的 观察T2DM患者血清血管生成素2(Ang-2)水平,探讨Ang-2与T2DM微血管病变的关系。方法 将64例T2DM患者分为单纯糖尿病(T2DM)组28例及糖尿病慢性肾脏疾病(CKD)组36例,另选健康对照(NC)组23名。结果 CKD组Ang-2高于T2DM组(2.38 vs 1.12 ng/ml,P<0.01),T2DM组Ang-2高于NC组(1.12 vs 0.46 ng/ml,P<0.01)。增殖性DR患者Ang-2高于非增殖性DR患者(2.83 vs 1.96 ng/ml,P<0.01),非增殖性DR患者Ang-2高于无DR患者(1.96 vs 1.08 ng/ml,P<0.05)。Ang-2与高敏C反应蛋白(hsC-RP)(r=0.63,P<0.01)、UAlb(r=0.29,P<0.05)呈正相关。结论 T2DM患者血清Ang-2水平升高,与hsC-RP、UAlb、DR病变颈动脉内中膜厚度呈正相关,Ang-2可作为评价T2DM患者肾脏和视网膜微血管病变的指标。

  【Abstract】Objective To observe the serum level of Angiopoietin-2 (Ang-2) in T2DM patients, and investigate the correlation between Ang-2 and microvascular complications. Methods In total the 64 2DM patients were involved, 28 of them without chronic kidney disease (CKD) were in the T2DM group, and 36 with CKD in the CKD group. The normal control (NC) group included 23 healthy subjects. Results the Ang-2 level was higher in the CKD group than in the T2DM group (2.38 vs 1.12 ng/ml, P<0.01), the Ang-2 level in the T2DM group was higher than in the NC group (1.12 vs 0.46ng/ml, P<0.01), the Ang-2 level was higher in the proliferative diabetic retinopathy (DR) patients than in the non-proliferative DR patients (2.83 vs 1.96ng/ml, P<0.01), and the Ang-2 level in the non-proliferative DR patients was higher than in the patients without DR (1.96 vs 1.08ng/ml, P<0.05). Ang-2 was positively correlated with high-sensitivity C-reactive protein (hsC-RP) (r=0.63, P<0.01) and UAlb (r=0.29, P<0.05). Conclusion T2DM patients have elevated Ang-2 level, which is positively correlated with hsC-RP, proteinuria, DR, and intima-media thickness (IMT). Ang-2 may be a potential biomarker of renal and retina microvascular involvement in T2DM patients.
