血清视黄醇结合蛋白4与2型糖尿病大血管病变的相关性研究

【摘要】目的研究血清视黄醇结合蛋白4(RBP4)与T2DM大血管病变的相关关系。方法测定T2DM合并大血管病变(MVC)组74例、未合并大血管病变(Non-MVC)组48例及健康对照(NC)组40名血清RBP4、TNF-α、IL-6、APN,计算胰岛素抵抗指数(HOMA-IR)。结果(1)T2DM患者血清RBP4水平显著高于NC组,MVC组较Non-MVC组明显增高(P<0.01);(2)相关性分析发现, RBP4与病程、BMI、TC、LDL-C、FIns、2 hIns、HOMA-IR、UAER、TNF-α呈正相关(P<0.05),与APN呈负相关(P<0.01 );(3)多元逐步回归表明LDL-C、HOMA-IR、APN、病程与血清RBP4之间有直线回归关系(P<0.05);(4)回归分析显示RBP4对大血管病变有一定作用。结论在T2DM患者中,血清RBP4增高与合并大血管病变有关。

【Abstract】ObjectiveTo investigate the correlation between serum retinol-binding protein 4 (RBP4) and macrovascular comp1ications of the T2DM patients.MethodsThe serum RBP4, TNF-α, IL-6, and APN were measured in 74 cases with macrovascular complications (MVC group), 48 cases without macrovascular complications (Non-MVC group), and 40 healthy people (NC group). The insulin resistance was evaluated by HOMA-IR.Results(1)The concentration of serum RBP4 was increased significantly in the T2DM group compared with that in the NC group, and it was significantly higher in the MVC group than that in Non-MVC group (P<0.01); (2)The analysis on the correlation between RBP4 and macrovascular complications of T2DM showed that the serum RBP4 was positively correlated with duration of diabetes, BMI, TC, LDL-C, FIns, 2 hIns, HOMA-IR, UAER, and TNF-α respectively (P<0.05), and negatively with APN (P<0.01);(3)The multiple regression analysis showed a straight regression correlation of LDL-C, HOMA-IR, APN, and duration of diabetes with the serum RBP4 (P<0.05);(4)The logistic regression analysis revealed an association between the serum RBP4 and marovascular comp1ications.ConclusionThe increase of RBP4 is correlated with macrovascular complications of T2DM patients.
