糖尿病肾病相关因素分析

【摘要】 目的 探讨糖尿病肾病(DN)的相关因素，为DN的一级预防提供理论依据。 方法 选择2型糖尿病(T2DM)患者267例，其中DN患者102例(DN组)，单纯糖尿病无肾病165例(NDN组)，采集血糖等临床资料，进行t检验、χ2检验和Logistic回归分析。 结果 组间比较显示，DN组与NDN组的年龄、糖尿病(DM)病程、BMI、吸烟、高脂饮食、合并高血压、FPG、2hPG、0.5hPG、HbA1c、TC、TG、LDL-C和纤维蛋白原(Fib)差异有统计学意义(P<0.05或P<0.01)。多元Logistic回归分析中，DN的发生与DM病程、TG、HbA1c、FPG、2hPG和合并高血压有关(P<0.05或P<0.01)。 结论 TG、DM病程、HbA1c、FPG、2hPG和合并高血压是DN发生的独立相关因素。

Analysis of relative factors of diabetic nephropathy

【Abstract】 Objective To investigate the relative factors of diabetic nephropathy. Methods In the enrolled 267 patients with type 2 diabetes mellitus, 102 had diabetic nephropathy (DN group) and 165 did not have (NDN group). The clinical and laboratory data were collected.The statistical analysis including t-test, Chi-square test and logistic regression analysis were completed. Results There were the statistical inter-group differences in age，course of diabetes，body mass index，smoking，high fat diet，hypertension，fasting plasma glucose，2 hours’ postprandial plasma glucose，0.5 hours’ postprandial plasma glucose，glycosylated hemoglobin A1C，total cholesterol，triglycerides，low density lipoprotein cholesterol and fibrinogen between two groups of T2DM with versus without diabetic nephropathy (P<0.05 or P<0.01).Multivariate logistic regression analysis showed diabetic nephropathy were related with course of diabetes，triglycerides， fasting plasma glucose，2-h postprandial plasma glucose and hypertension(P<0.05 or <0.01). Conclusion Course of diabetes，triglycerides，glycosylated hemoglobin A1C，fasting plasma glucose，2-h postprandial plasma glucose and hypertension are the independent relative factors of diabetic nephropathy.
