2型糖尿病患者游离三碘甲状腺原氨酸与血脂代谢关系的研究

       【摘要】  目的  评估不同血糖控制情况下T2DM患者甲状腺功能对血脂代谢的影响。  方法  选取住院T2DM患者595例,根据HbA1c水平分为HbA1c7.0%108例、7.0%≤HbA1c9.0%164例及HbA1c≥9.0%323例,另选取健康体检者61名作为对照(NC)组。对各组指标进行比较,明确T2DM患者甲状腺功能指标与非高密度脂蛋白胆固醇(non-HDL-C)的关系。  结果  

HbA1c≥9.0%non-HDL-C水平高于HbA1c7.0%7.0%≤HbA1c9.0%组和NC[3.84±1.16vs3.32±1.07),3.59±0.96),(3.21±0.77mmol/LP0.05]HbA1c≥9.0%组游离三碘甲状腺原氨酸(FT3)水平低于HbA1c7.0%7.0%≤HbA1c9.0%NC[2.43±0.50vs 2.69±0.46),(2.66±0.45),(2.91±0.44pg/mlP0.05]T2DM患者non-HDL-CBMIFPGHbA1c呈正相关,与FT3呈负相关。多元线性回归分析提示,BMIHbA1cFT3均是non-HDL-C的影响因素。  结论   随血糖升高,T2DM患者血脂代谢紊乱加重,FT3水平的下降,FT3是影响T2DM患者血脂变化的重要因素。

       The study of relationship between serum free tri-iodothyronin and the lipid metabolism in patients with type 2 diabetes mellitus

       Abstract   Objective   To investigate the impact of thyroid function on lipid metabolism in type 2 diabetes mellitus (T2DM) patients stratified by blood glucose control.   Methods    A total of 595 T2DM patients were enrolled and divided into three groups according to HbA1c levelHbA1c7.0% (n=108), 7.0%≤HbA1c9.0% (n=164), HbA1c≥9.0% (n=323). 61 healthy subjects were set as control (NC group). All subjects underwent assessment of thyroid function, glucose and lipid metabolism.   Results   HbA1c≥9.0% group had higher levels of non-HDL-C and lower levels of free tri-iodothyronin (FT3) than HbA1c7.0% group, 7.0%≤HbA1c9.0% group and NC group [non-HDL-C:3.84±1.16vs 3.32±1.07, 3.59±0.96, 3.21±0.77);FT3:(2.43±0.50pg/ml vs 2.69±0.46, 2.66±0.45, 2.91±0.44pg/ml, P0.05respectively]. Non-HDL-C was positively correlated with BMI, FPG and HbA1c, and negatively correlated with FT3 in T2DM patients. Multiple linear regression analysis showed that BMI, HbA1c and FT3 were influence factors of non-HDL-C. Conclusions  The disorder of lipid metabolism showed a deteriorating trend with the increase of blood glucose. FT3 was an influence factor of lipid metabolism in T2DM patients.
