糖代谢异常患者血清脂肪细胞脂肪酸结合蛋白的变化及其临床意义

【摘要】  目的  观察不同血糖水平血清脂肪细胞脂肪酸结合蛋白(A-FABP)水平,探讨其与IR的相关性。 方法  选取NGT组55名、IGT组53例及T2DM组56例,测定各组血清A-FABP、FPG、2 hPG、TC、TG、FIns等指标,计算BMI,WHR及胰岛素抵抗指数(HOMA-IR),分析A-FABP与IR的相关性。 结果 T2DM组血清A-FABP水平高于IGT组和NGT组[(71.14±10.03)vs(36.69±8.01)vs(19.49±2.98)mg/L,P<0.05]。相关分析显示,血清A-FABP水平与BMI、WHR、TC、TG、HDL-C、LDL-C、HbA1c、2 hPG、高敏C反应蛋白(hsC-RP)、HOMA-IR相关(r=0.739、0.320、0.202、0.213、-0.695、0.248、0.842、0.441、0.354、0.695,P<0.05或P<0.01)。多元回归逐步分析显示,BMI、WHR、HbA1c、HOMA-IR是影响血清A-FABP的独立危险因素(t=2.621、4.461、7.362、3.875,P<0.05)。  结论  A-FABP与IR关系密切,随着IR程度的加重,A-FABP水平逐渐升高。

Abstract Objective To observe serum adipocyte-fatty fatty acid binding protein (A-FABP) level and its association with insulin resistance in patients with different glucose tolerance.  Methods  A total of 164 patients were enrolled in this study and divided into three groups: normal glucose tolerance (NGT group, n=55), impaired glucose tolerance (IGT group, n=53) and type 2 diabetes mellitus (T2DM group, n=56). A-FABP, FPG, 2 hPG, TC, TG, FIns were tested in all subjects. BMI, WHR and HOMA-IR were calculated and the relationship between A-FABP and IR was analyzed.   Results   A-FABP level was higher in T2DM group than in IGT and NGT group [(71.14±10.03)vs(36.69±8.01)vs(19.49±2.98)mg/L,P<0.05]. Association analysis showed that serum A-FABP was positively correlated with BMI、WHR、TG、TC、LDL-C、HbA1c、2 hPG、hsC-RP and HOMA-IR(r=0.739,0.320,0.202,0.213,0.248,0.842,0.441,0.354,0.695, P<0.05 or P<0.01),but was negatively correlated with HDL-C(r=-0.695,P<0.01). Multiple regression analysis showed that BMI、WHR、HbA1c、HOMA-IR were independent influencing factors for A-FABP (t=2.621,4.461,7.362,3.875,P<0.05).  Conclusion  A-FABP is closely related to IR. The level of A-FABP rises with the increase of IR severity.
