血清高分子量脂联素水平与代谢综合征及其组分的相关性研究

      【摘要】 目的探讨血清高分子量脂联素(HMw-APN)水平与MS及其组分的相关性。 方法   根据2005年IDF的MS诊断标准将受试者分为MS组69例和非MS(Non-MS)组69例,采用ELISA检测各组血清HMW-APN浓度;再根据血清HMW-APN水平三分位水平分为HMW APN<1.36 mg/L组、1.36 mg/L≤HMW-APN<2.45 mg/L组和HMW-APN≥2.46 mg/L组,检测各组各项临床指标。结果    MS组血清HMW-APN水平低于Non-MS组[2.82(1.82,5.55)vs 1.16(0.72,1.66)mg/L,P<0.01]。单相关性分析显示,血清HMW-APN水平与HDI1c、胰岛8细胞功能指数(HOMA-p)呈正相关,与WC、颈围(NC)、BMI、WHR、SBP、FPG、2 hPG、Fins、2 hIns、HbAlC、TG、TC和胰岛素抵抗指数(HOMA-β)呈负相关。多元线性回归分析结果显示,WC、2 hPG和SBP是血清HMW-APN水平的独立影响因素。HMW-APN<1.36 mg/L组、1.36 mg/L≤HMW-APN<2.45 mg/L组和HMW-APN≥2.46组MS患病率分别为86.9%、52.1%和10.患病率降低,提示HMW-APN在MS发生发展中发挥重要作用。

      【Abstract】  Objective   To investigate the relationship between serum high-molecular-weight adiponectin(HMW-APN)1evel with MS and its components. Methods    A total of 138 Chinese subjects were selected and divided into MS group(n=69)and Non-MS group (n=69) according to MS diagnostic criteria of 2005 International Diabetes Federation.Serum HMW-APN concentration was determined bv ELISA.A1l subjects were also divided into three subgroups according tO serum adiponectin concentration:
HMW-APN<1.36 mg/L,1.36 mg/L≤HMW APN<2.45 mg/L and HMW-APN≥2.46 mg/L respectively.Clinical parameters were measured in each subgroup. Results    Serum HMW-APN was significantly declined in MS group than in non-MS group[2.82(1.82,5.55)vs 1.16(0.72,1.66)mg/L,P<0.01].Correlation analysis showed that serum HMW-APN concentration was positively correlated with HDL-C and HOMA-13,but negatively correlated with WC,NC,BMI,WHR,SBP,FPG,2 hPG,
Fins,2 hIns,HbA1c,TG,TC and HOMA_IR.Multiple stepwise 1inear regression analysis showed that WC.2 hPG and SBP were independent influencing factors for serum Hmw-APN concentration.The morbidity of MS in HMW—APN<1.36 mg/L group,

1.36 mg/L≤HⅣmLAPN<2.45 mg/L group and HMW-APN≥2.46 mg/L group were 86.9%,52.1% and 10.9%respectively

(P<0.05). Conclusion   The morbidity of MS became 10wer when serum Hmw-APN level inereased.HMW-APN may play a
critical part in the pathogenesis of MS.
