【摘要】 目的 研究非老年男性糖尿病慢性肾脏疾病(CKD)不同阶段骨代谢指标的变化。 方法 将160例非老年男性T2DM患者分为正常蛋白尿组、微量白蛋白尿组和临床蛋白尿组,另选30名非老年健康男性作为正常对照组,测定血清骨钙素(OC)、Ⅰ型前胶原N末端前肽(PINP)和β胶原特殊序列(β-Crosslaps)水平并进行分析。 结果 微量白蛋白尿组OC、PINP、β-Crosslaps水平较正常对照组降低[(12.12±4.47) vs (16.05±5.68)ng/ml、(33.19±12.04) vs (41.90±11.56)ng/ml、(0.26±0.12) vs (0.36±0.14)ng/ml,P<0.05];微量白蛋白尿组PINP水平较正常蛋白尿组降低[(40.51±15.94)ng/ml,(P<0.05)]。 结论 非老年男性CKD早期骨代谢以低转换为主,后期逐渐转化为高转换。

【Abstract】 Objective To study the feature of bone metabolism in different stages of non-elderly men with type 2 diabetes mellitus with chronic kidney disease (CKD). Methods A total of 160 cases non-elderly male patients with CKD were enrolled and randomly divided into normal albuminuria group, microalbuminuria group and macroalbuminuria group, while 30 non-elderly healthy men were selected as normal control group. The levels of serum osteocalcin (OC), procollagen type I N propeptide (PINP) and β-Crosslaps were measured for statistical analysis. Results Compared with normal control group, the levels of serum OC, PINP and β-Crosslaps were significantly lower in microalbuminuria group [(12.12±4.47) vs (16.05±5.68) ng/ml, (33.19±12.04) vs (41.90±11.56) ng/ml, (0.26±0.12) vs (0.36±0.14) ng/ml, respectively, P<0.05]; and compared with normal albuminuria group, the level of serum PINP was significantly lower in microalbuminuria group [(33.19±12.04) vs (40.51±15.94) ng/ml,P<0.05]. Conclusions Clinical effect on bone metabolism differs from stages in non-elderly male patients with CKD,which is mainly low turnover in early phase and gradually transformed into high turnover in the end.
