糖尿病与肥胖、脂代谢异常

　　【摘要】目的探讨宁夏回汉民族血清热应激蛋白70(HSP70)表达水平与T2DM发病的关系。方法应用病例对照研究法收集T2DM患者(T2DM组)201例，另选健康体检者(NC组)201名，采用ELISA检测两组血清HSP70的表达水平。 结果 T2DM组血清HSP70蛋白表达水平明显高于NC组[(0.147±0.126) vs(0.110±0.113) ng/ml](t=3.066，P=0.002)。血清HSP70高表达者患T2DM的风险是低表达者的2.307倍。回族人群血清HSP70表达水平高于汉族人群[(0.145±0.130)vs(0.114±0.111)ng/ml](t=2.591，P=0.010);回族人群HSP70高表达的风险是汉族人群的1.760倍。腹型肥胖、非腹型肥胖T2DM组与NC组血清HSP70表达水平[(0.152±0.128)vs(0.128±0.119) vs(0.111±0.121)ng/ml]差异有统计学意义(F=5.335，P=0.005)。 结论 T2DM人群中血清HSP70表达水平明显高于健康人群，高表达者患T2DM的风险高于低表达者。回族人群血清HSP70表达水平高于汉族，回族人群高表达的风险高于汉族。腹型肥胖T2DM人群血清HSP70表达水平高于正常人群。血清HSP70水平可能是影响T2DM患病的重要因素之一。

　　【Abstract】 Objective To investigate the relationship between serum heat shock protein 70 and type 2 diabetes mellitus in Moslem and Han people in Ningxia. Methods According to case-control study， 201 patients with type 2 diabetes (T2DM group) and 201 healthy cases (NC group) tested in Wuzhong People’s Hospital and Second Affiliated Hospital of Ningxia Medical University from February 2009 to December 2010 were enrolled in the study.The expression levels of serum heat shock protein 70 in two groups were detected by ELISA. Results The expression of serum HSP70 was significantly higher in T2DM group than that in control group [(0.147±0.126) ng/ml vs (0.110±0.113) ng/ml] respectively. And the risk of T2DM in patients with higher expression of serum HSP70 was 2.037 times of patients with lower expression . The expression level of HSP70 was significantly higher in Hui people than in Han people[(0.145±0.130)vs (0.114±0.111)ng/ml](t=2.591，P=0.010). And compared with Han people, we foundOR valueof Hui people was 1.760. There was statistically significant difference in the expression levels of HSP70 among abdominally obese, non-abdominally obese T2DM group and control group[(0.152±0.128)vs(0.128±0.119) vs(0.111±0.121)ng/ml](F=5.335，P=0.005). Conclusions The expression level of serum HSP70 was obviously higher in T2DM group than in healthy people, and the risk of T2DM in people with high expression level of serum HSP70 was higher than people with lower expression; serum HSP70 expression level was significantly higher in Hui people than in Han people, and the risk of high serum HSP70 expression level was higher in Hui than in Han ; serum HSP70 expression level was higher in abdominally obese people than in normal people. Therefore, HSP70 may be one of the important factors influencing the onset of T2DM, which may be concerned with abnormal high expression of HSP70 that reduces the stability and adaptability of intracellular environment. And further research is required for that whether it could be took as the parameters of the onset, clinical treatment and prognosis judgment T2DM and for the exact role of HSP70 on the pathogenesis of T2DM and its mechanism.
