【摘要】 目的 探讨快速血酮与尿酮体检测对糖尿病酮症和DKA诊断及治疗的临床意义。 方法 选取糖尿病酮症(有/无酸中毒)患者(DKA组)50例,糖尿病无酮症患者(NDK组)100例及健康体检者(NC)100名。检测治疗前各组和治疗后DKA组末梢血β-羟丁酸(β-HBA)、FPG及尿酮体,并进行比较分析。 结果 DKA、NDK和NC组末梢血β-HBA检测阳性率分别为100%、12%、0%,尿酮体检测阳性率分别为88%、6%、3%。β-HBA浓度与FPG浓度呈正相关性。DKA组治疗后末梢血β-HBA检测阳性率0%,尿酮体检测阳性率10%。 结论 末梢血β-HBA检测较尿酮体检测灵敏度更高,准确度更好,对糖尿病酮症及其DKA的早期诊断和病情评估有意义。

【Abstract】 Objective To study the diagnosis and treatment clinical meaning of rapid blood ketone detection and urine ketone bodies detection on diabetic ketosis and diabetic ketoacidosis. Methods 50 diabetic ketosis patients with or without acidosis were selected as DKA group, 100 diabetes patients without ketosis as NDK group and 100 healthy people as normal control (NC) group. The peripheral blood β-hydroxybutyric acid (β-HBA), FBG and urine ketone bodies before treatment in three goups and after treatment in DKA group were tested, then the results were compared and analyzed. Results The positive rate of β-HBA in DKA, NDK and NC group was 100%, 12% and 0%, respectively. The positive rate of urine ketone body in DKA, NDK and NC group was 88%, 6%, 3%, respectively. There was a significant positive correlation between the concentration of β-HBA and blood glucose. After treatment, the positive rate of β-HBA and urine ketone body in DKA group was 0%, and 10%. Conclusion The sensitivity and accuracy of the peripheral blood β-HBA detection is better than the urine ketone body detection, and it have important significance for early diagnosis and assessment in diabetic ketosis and diabetic ketoacidosis.
