【摘要】 目的 评价末梢血β-羟丁酸在DKA诊断中的价值。 方法 检索Pubmed、ScienceDirect、EMBase、CNKI、维普和万方数据库关于末梢血β-羟丁酸对DKA诊断价值的文献。采用诊断性试验准确性质量评价工具(QUADAS)评价纳入文献质量，并应用Meta Disc1.4软件行综合定量评价。 结果 最终纳入7篇文献。末梢血β-羟丁酸在DKA中的诊断价值合并敏感度为0.96(95%CI：0.92～0.98)，合并特异度为0.88(95%CI：0.86～0.89)，合并为诊断优势比为153.01(95%CI：34.70～674.73)，AUCSROC=0.9304(SE=0.0251)，Q指数为0.8658(SE=0.0302)。 结论 末梢血β-羟丁酸诊断DKA的敏感度、特异度均较高，AUCSROC>0.9，诊断准确性高，适于临床推广应用。

　　【Abstract】 0bjective To assess the diagnostic value of capillary beta-hydroxybutyrate for diabetic ketoacidosis(DKA) diagnosis. Methods Pubmed, ScienceDirect, EMBase, CNKI, VIP and WANFANG were searched to get all diagnostic tests of capillary beta-hydroxybutyrate for DKA. QUADAS items were used to evaluate the quality of the eligible studies. Meta disc 1.4 software was used to perform comprehensive quantitative assessment for all included studies and systematic ROC(SROC) were drawn. Results A total of 7 studies were included, all of which got 10 scores in QUADAS.The outcomes of the diagnostic value of capillary beta-hydroxybutyrate for DKA were as follows：pooled sensitivity was 0.96(95%CI:0.92～0.98), pooled specificity was 0.88(95%CI：0.86～0.89), pooled diagnostic odds ratio(DOR) was 153.01(95%CI：34.70～674.73), AUCSROC is 0.9304(SE=0.0251), and Q index was 0.8658(SE=0.0302). Conclusions The diagnostic sensitivity and specificity of capillary beta-hydroxybutyrate are pretty high for DKA, and the AUCSROC is greater than 0.9, which indicates a high diagnostic accuracy. This test shoud be recommended to apply in diagnosing DKA.
