【摘要】 目的 探讨DN患者血清中性粒细胞明胶酶相关脂质运载蛋白(NGAL)的变化及临床意义。 方法 T2DM患者99例根据UAER分为 3 组,即正常白蛋白尿(NA)组32例、微量白蛋白尿(MA)组39 例及临床肾病(CN)组28例;同时选取 30 名健康体检者作为对照(NC)组。应用 ELISA 法检测血清 NGAL 水平,同时测BMI、FPG、HbA1c、高敏 C 反应蛋白(hs-CRP)、血脂、UAER,比较各组间的差异。 结果 T2DM患者各组血清 NGAL 水平均较NC组明显升高(P < 0.01)。相关分析显示,血清 NGAL 水平与 Cr、UAER、hs-CRP、CHO、HDL-C 成正相关,r 分别为 0.664、0.877、0.726、0.300、0.578,P < 0.01;与 FPG 成正相关,r 为 0.180,P < 0.05;与 HbA1c成负相关,r 为 -0.172,P < 0.01。多元逐步回归分析显示,与血清 NGAL 水平密切相关的因素为 UAER、hs-CRP。 结论 DN患者血清 NGAL 水平明显升高,且血清 NGAL 水平与 UAER、hs-CRP 密切相关。提示 NGAL 与早期肾损害程度相关。

【Abstract】 0bjective To investigate the changes and clinical significance of serum neutrophil gelatinase-associated lipocalin (NGAL) in patients with diabetic nephropathy (DN). Methods A total of 99 T2DM patients and 30 healthy people as control were enrolled in the study. According to UAER, the diabetic patients were divided into groups NA (normal albuminuria, n = 32), MA (microalbuminuria, n= 39), and CN (clinical nephropathy, n = 28). NGAL level was detected by ELISA. BMI, FPG, HbA1c, hs-CRP, blood lipid, and UAER were measured as well. Data were compared among groups. Results The serum NGAL level was higher in T2DM patients than that in the healthy control group (P< 0.01). Correlation analysis indicated that serum NGAL level was positively correlated with Cr (r=0.664), UAER (r=0.877), hs-CRP (r=0.726), CHO (r=0.300), and HDL-C (r=0.578) (P<0.01), and FPG (r=0.180) (P< 0.05); while negatively with HbA1c (r=-0.172) (P<0.01). Multiple regression analysis showed that UAER and hs-CRP were closely associated with the level of serum NGAL. Conclusion The serum NGAL level in patients with DN significantly increases and is closely correlated with UAER and hs-CRP. It suggests an association of NGAL level with early renal damage.
