2型糖尿病患者血清尿酸水平与踝肱指数相关性的研究

  【摘要】 目的 探讨T2DM患者血清尿酸(UA)水平与踝肱指数(ABI)的相关性。方法 选取经住院确诊的T2DM患者873例,根据UA水平分为4组:Q1组(<240 µmol/L)181例,Q2组(240~300 µmol/L)162例,Q3组(301~360 µmol/L)164例和Q4组(>360 µmol/L)366例。根据ABI分为2组:无大血管病变组(ABI≥0.9)663例和大血管病变组(ABI<0.9)210例。 结果 Q1~Q4组间年龄、病程、SBP、HbA1c、TC、TG、尿白蛋白(UAE)、Scr、肾小球滤过率(eGFR)和ABI差异均有统计学意义(P均<0.05)。UA与病程、BMI、SBP、TG、UAE、Scr、ABI间均呈正相关关系(P均<0.05),与HbA1c、eGFR呈负相关(P均<0.05)。与Q1组相比,Q2、Q3与Q4组(P=0.097,0.023,0.005)发生大血管病变的风险增加(趋势P=0.041),校正相关危险因素后,虽然OR逐渐减少,但趋势P值<0.05。 结论 T2DM患者血清UA水平与ABI具有相关性,血清UA高水平可能是T2DM患者发生大血管病变的危险因素。

  【Abstract】 Objective To investigate the relationship between serum uric acid (UA)levels and ankle-brachial index (ABI) in T2DM patients. Methods A total of 873 T2DM subjects were enrolled in this study. They were divided into 4 groups according to the UA levels, including Q1 group (<240µmol/L, n=181), Q2 group (240~300µmol/L, n=162), Q3 group (301~360µmol/L, n=164) and Q4 group (>360µmol/L, n=377). However, they were divided into 2 groups according to ABI, namely, without macroangiopathy group (ABI<0.9, n=663) and with macroangiopathy group (ABI≥0.9, n=210). Results There were statistically significant differences in age, duration, systolic blood pressure (SBP), HbA1c, total cholesterol (TC), triglycerides (TG), serum creatinine (Scr), urinary albumin (UAE), estimated glomerular filtration rate (eGFR), and ABI among the Q1~Q4 groups (P < 0.05). The UA level was significantly positively correlated with duration, BMI, SBP, TG, UAE, Scr, and ABI, while negatively correlated with HbA1c and eGFR. Compared to Q1 group, there was significantly increased risk of macroangiopathy in Q2 group (P=0.097), Q3 group (P=0.023), and Q4 group (P=0.005). After adjusting the related risk factors of sex, age, BMI, duration, SBP, HbA1c, TC, TG, and eGFR, the ORs were attenuated but remained significant (P for trend <0.05). Conclusion The serum uric acid level in T2DM subjects was significantly associated with ABI. The high UA level may be one of the risk factors for macroangiopathy in T2DM patients.
