二甲双胍和/或格列吡嗪对2型糖尿病患者8-异-前列腺素F2α及血糖水平影响的观察

        【摘要】 目的 探讨二甲双胍格列吡嗪复方制剂与单药二甲双胍或格列吡嗪对T2DM患者氧化应激水平的影响。 方法 将90例T2DM患者为三组,A组35例,予二甲双胍格列吡嗪复方制剂;B组30例,予二甲双胍片;C组25例,予格列吡嗪片,疗程均为12周。治疗前后均检测FBG、2 hPG、HbA1c、FIns及2 hIns水平,计算HOMA-IR、HOMA-β。试验前后留取血清测定血清8-异前列腺素2α(8-iso-PGF2α)。 结果 三组治疗后血清8-iso-PGF2α水平均有下降,仅A组治疗前后差异有统计学意义(P<0.001);A、C组HbA1c水平治疗前后差异有统计学意义(P<0.001);A、C组FPG、2 hPG水平降低(P<0.05);A组治疗前后HOMA-IR差异有统计学意义(P<0.01)。Pearson相关性分析显示8-iso-PGF2α水平与2 hPG、2 hIns、HbA1c、HOMA-IR呈相关关系(r=0.199,P<0.05;r=0.206,P<0.05;r=-0.276,P<0.01;r=-0.451,P<0.002)。 结论 二甲双胍格列吡嗪复方制剂既能良好控制患者血糖,亦能显著改善患者氧化应激水平,从而间接改善患者的血糖波动,从而有效控制患者并发症的发生。

  【Abstract】 Objective To explore the effect of compound and single Glipizde/Metformin on 8-iso-PGF2α and oxidative stress of T2DM patients. Methods Ninety T2DM patients were divided into 3 groups. Group A, n=35, was treated with Compound Glipizde/Metformin Tablet; Group B, n=30, treated with Metformin Tablet; and Group C, n=25, treated with Glipizde Tablet. The treatment was of 12 weeks. Before and after treatment, the fasting blood glucose (FBG), postprandial blood glucose (2 hPG), glycosylated hemoglobin (HbA1c), fasting insulin (FIns), and postprandial insulin (2 hIns) were all tested for calculating HOMA-IR and HOMA-β, and the serum 8-iso-PGF2α was determined too. Results Reduction of 8-iso-PGF2α was showed in all 3 groups, however, only Group A had a significant reduction (P<0.001). The difference in HbA1c levels of Group A and C was significant (P<0.001). The levels of FPG and 2hPG in Group A and C were reduced (P<0.05). The difference of HOMA-IR after treatment in Group A was significant (P<0.01). The Pearson correlation analysis showed that the level of 8-iso-PGF2α was related with 2 hPG, 2 hIns, HbA1c, and HOMA-IR (r=0.199,P<0.05; r=0.206, P<0.05; r=-0.276, P<0.01; r=-0.451, P<0.002). Conclusion Compound Glipizde/Metformin Tablet could control blood glucose and improve oxidative stress of the T2DM patients. Through the way of reducing the fluctuation of blood glucose, the development of complications is thus delayed.
