网膜素与2型糖尿病非酒精性脂肪肝病的病例对照研究

　　【摘要】 目的 分析网膜素(omentin)与T2DM非酒精性脂肪肝病(NAFLD)发生的关系。 方法 采用ELISA测定190例初发T2DM患者 (不合并脂肪肝102例为A组;合并脂肪肝88例为B组)空腹血浆omentin水平，比较2组各代谢参数，并对各变量进行相关分析和Logistic回归分析。 结果 B组空腹血浆omentin水平显著低于A组(P<0.05)，(53.67±21.84) ng/ml vs (70.74±21.17) ng/ml。在T2DM患者中血浆omentin水平与FIns、HOMA-IR、WC、FPG、2hPG呈负相关(P<0.05)，与HDL-C呈正相关(P<0.05)。Logistic回归分析结果显示omentin作为独立因素影响NAFLD。由受试者工作特征即ROC曲线得到omentin预测脂肪肝的大致风险值为40ng/ml。 结论 omentin可能影响T2DM患者NAFLD的发生发展。

　　【Abstract】 Objective To investigate the relationship between the level of plasma omentin and onset non-alcoholic fatty liver disease (NAFLD) in Type 2 diabetes mellitus. Methods The level of fasting plasma omentin of incipient T2DM patients (88 with fatty liver disease, group B; 102 without fatty liver disease, group A) was assayed by enzyme-linked immunosorbent assay (ELISA). The metabolic indexes of the two groups were compared, the correlation of variables was analyzed, and the Logistic regression analysis was made in the trial. Results The level of fasting plasma omentin in group B was significantly lower than that in group A (P<0.05), (53.67±21.84)ng/ml vs (70.74±21.17)ng/ml. The analysis of correlation of variables showed that the level of fasting plasma omentin was in a negative correlation with FIns, HOMA-IR, WC, FPG, and 2hPG (P<0.05) and in a positive correlation with HDL-C (P<0.05). Logistic regression analysis indicated that omentin was an independent factor that influenced NAFLD. The risk value of NAFLD of omentin prediction obtained from ROC was 40ng/ml. Conclusion Omentin may contribute to the development of NAFLD in T2DM.
