【摘要】 目的 比较评价糖尿病患者HbA1c和FPG联合测定的临床价值。方法 选取湖北省十堰地区为明确诊断的681例门诊患者，使用- 1 -Bio-RAD-10糖化血红蛋白分析仪检测HbA1c，OLYMPUS AU-5400全自动生化分析仪检测FPG，经0GTT复查确诊糖尿病，对其相关性进行分析。结果 681例门诊患者同时测定HbA1c和FPG，确诊114例糖尿病患者。其中30例FPG正常，HbA1c异常，最后确诊26例为T2DM患者;92例FPG和HbA1c均异常的患者，确诊88例糖尿病患者。24例FPG异常，HbA1c正常，确诊3例FPG异常的FPG受损，2例FPG异常的糖耐量减低;FPG正常，HbA1c异常6例，确诊3例糖耐量减低。结论 同时测定HbA1c和FPG可提高对糖尿病的诊断和筛选特异性和敏感性，可提高糖耐量减低及轻度T2DM的早期诊断。

　　【Abstract】 object To evaluat clinical value of glycosylated hemoglobin (HbA1c) and fasting plasma glucose (FPG) combined determination in patients with diabetes mellitus (DM). Methods: 681 patients with DM were all from Shiyan, the level of HbA1c was determined by Bio-RAD-10 glycated hemoglobin analyzer, FPG were determined by OLYMPUS AU-5400 automatic biochemistry analyzer, and reviewed by OGTT for final diagnosis, then analysis the correlation. Results: 681 cases of clinic patients simultaneous determination of HbA1cand FPG, 114 were diagnosed as diabetes. 26 of 30 FPG normal but HbA1c abnormal cases eventually proved as type 1 diabetes, 88 of 92 with both HbA1c and FPG abnormal cases proved as type 2 diabetes. Of 24 cases FPG abnormal but HbA1c normal cases, 3 were diagnosed as HbA1c、FPG abnormal IFG (impaired fasting glucose), 2 were FPG abnormal impaired glucose tolerance (IGT decrease). And among 6 FPG normal but HbA1c abnormal patients, 3 were diagnosed as IGT. conclusion: simultaneous determination of HbA1c and FPG can improve the diagnoses of diabetes mellitus and screening specificity and sensitivity, at the same time it can improve impaired carbohydrate tolerance and diagnosis minor type Ⅱ diabetes in early time.
