瑞格列奈对2型糖尿病患者早时相胰岛β细胞功能影响的临床研究

  【摘要】 目的 探讨瑞格列奈联合口服葡萄糖耐量试验—胰岛素释放试验(OGTT-IRT)评价新诊断的2型糖尿病(T2DM)患者胰岛β细胞功能的价值。 方法 96例新诊断的T2DM患者在入组时行OGTT试验,其结果作为基线资料。将患者随机分为3组,A组服单次瑞格列奈行OGTT,B组服1周瑞格列奈行OGTT,C组服1周格列本脲行OGTT。测定各时刻真胰岛素(TI)和免疫反应性胰岛素(IRI),并计算TI/IRI及早时相胰岛素分泌指数ΔI30/ΔG30。 结果 A组和B组服药后30 min时TI和TI/IRI较基线明显增加(P<0.05),ΔI30/ΔG30均较基线显著升高(P<0.01);C组服药前后上述指标均无统计学差异。 结论 瑞格列奈改善早时相胰岛素分泌,其联合OGTT-IRT较常规OGTT-IRT能更真实反映新诊断的T2DM患者胰岛β细胞分泌和储备功能。

  The clinical study on effect of repaglinide on early phase β-cell function in patients with newly diagnosed type 2 diabetes mellitus

  【Abstract】 Objective To explore the effect of repaglinide on early phase β-cell function in patients with newly diagnosed T2DM. Methods 96 newly diagnosed T2DM patients were randomly divided into 3 groups. As a baseline, OGTT was performed at admission, and was repeated 15 minutes after 2.0 mg repaglinide was taken on the second day’s morning in group A. In group B, OGTT was performed after taking repaglinide for 7 days. In group C, OGTT was performed after taking glibenclamide for 7 days. True insulin (TI) was measured with ELISA, and immunoreactive insulin (IRI) with radioimmunoassay. The ratios of TI/IRI and ΔIns30/ΔG30 were calculated. Result The levels of TI and TI/IRI ratio in groups of A and B were higher at the 30-minute post-treatment evaluation than at baseline, so was the ΔIns30/ΔG30 ratio (all P<0.05). There was no statistical difference between pre- and post-treatment in group C. Conclusion Repaglinide can improve the early phase insulin release significantly (P<0.05).
