糖调节受损人群胰岛β细胞功能与增龄的相关性研究

  【摘要】 目的 调查上海市中老年糖调节受损(IGR)人群胰岛β细胞功能并对相关因素进行分析。 方法 对大宁路街道的588名小区居民进行糖耐量筛查,测定生化指标,并进行统计分析。 结果 共检出IGR 90例(15.3%),其中单纯空腹血糖受损(IFG) 9例(10%)、单纯糖耐量减低(IGT) 66例(73.3%)、IFG+IGT 15例(16.7%)。对资料完整的60例患者按年龄分为A组(45~49岁)、B组(50~59岁)、C组(60~69岁)、D组(≥70岁)。A、B组FIns、2hIns水平高于C、D组。A、B组胰岛素抵抗指数(HOMA-IR)、胰岛素分泌功能指数(HOMA-β)高于C、D组。年龄与FIns、2hIns水平负相关。A、B组血清高敏C反应蛋白(hsC-RP)水平低于C、D组。 结论 随着年龄的增长,胰岛β细胞功能逐渐减退,导致糖耐量减低发生;亚临床炎症可能是其发生机制之一。

  The relationship between pancreatic β cell function and aging in patients with impaired glucose regulation

  【Abstract】 Objective To investigate the relationship between aging and islet β-cell function in patients with impaired glucose regulation in Shanghai. Methods We performed oral glucose tolerance screening tests in 588 people aged over 45 years on Daning community residents of Shanghai. A detailed analysis of biochemical parameters was carried out. Results 90 cases (90/588, 15%) had impaired glucose regulation with 9 cases (10%) of impaired fasting glucose(IFG), 66 cases (73%) of impaired glucose tolerance (IGT) and 15 cases (17%) of IFG plus IGT. The 60 cases with intact data were classified into 4 groups: A group aged 45-49 years; B group 50-59 years; C group 60-69 years; D group over 70 years. The fasting and 2-h postprandial insulin level was higher in A and B group than in C and D group. The HOMA-IR and HOMA-β were higher in A and B group than in C and D group. Age negatively correlated to serum fasting and 2-h postprandial insulin level. The serum high-sensitivity CRP of A、B group was lower than that of C、D group. Conclusion Along with aging, pancreatic β-cell function declines gradually and leads to IGR.
