【摘要】 目的 探讨糖耐量正常的腹型肥胖25~44岁男性睾酮、25(OH)D3水平及其与IR的关系。 方法 选取糖耐量正常的25~44岁男性129名,根据WC分为腹型肥胖组和腰围正常组,检测两组血清总睾酮(TT)、25(OH)D3及其他生化指标,分析睾酮水平下降的危险因素。 结果 腹型肥胖组胰岛素抵抗指数(HOMA-IR)较腰围正常组高[2.18(1.33,3.21) vs 1.13(0.34,1.62),P<0.01],TT[(6.12±0.24) vs (7.50±0.63)ng/ml,P<0.01]、25(OH)D3[(39.89±5.34) vs (51.91±8.72) mmol/L,P<0.01]低。腹型肥胖组TT与HDL-C、25(OH)D3呈正相关,与WC、FIns、HOMA-IR呈负相关。多元线性回归分析显示,25(OH)D3、HOMA-IR与TT相关性最密切。 结论 糖耐量正常的腹型肥胖25~44岁男性已出现IR和TT、维生素D(VitD)水平下降,VitD水平下降和IR与睾酮水平下降的相关性最密切。

【Abstract】 Objective To investigate the changes of testosterone and 25(OH)D3 level and its relationship with IR in obese men with NGT. Methods 129 NGT men aged 25~44 years were divided into abdominal obesity group and normal waist circumference group. Total testosterone (TT), 25(OH)D3 and other indexes were measured. The risk factors for TT decrease were analyzed using multiple linear regression. Results Compared with normal waist circumference group, abdominal obesity group showed the increased level of HOMA-IR [2.18(1.33,3.21) vs 1.13(0.34,1.62), P<0.01], and the decreased levels of TT [(6.12±0.24) vs (7.50±0.63)ng/ml, P<0.01] and 25(OH)D3 [(39.89±5.34) vs (51.91±8.72)mmol/L, P<0.01]. In abdominal obesity group, WC, FIns, HOMA-IR showed a negative correlation with TT, and HDL-C and 25(OH)D3 were positively correlated with TT (P<0.05). The level of 25(OH)D3 and HOMA-IR had an independently positive correlation with TT. Conclusion Abdominal obese men aged 25~44 years with NGT have IR and decreased TT and vitamin D (VitD). IR and VitD deficiency may play an important role in decrease of testosterone.
