新诊断酮症倾向2型糖尿病患者肌酶谱和肌红蛋白水平的变化

  【摘要】 目的 探讨新诊断酮症倾向T2DM患者肌酶谱和肌红蛋白(Mb)水平的变化。 方法 选取新诊断酮症倾向T2DM患者(K-T2DM组)97例、非酮症倾向T2DM患者(T2DM组)49例和健康对照者(NC组)48名,检测各组肌酶谱、Mb及其他生化指标,分析肌酶谱和Mb的影响因素。 结果 K-T2DM组较NC组肌酸激酶(CK)、肌酸激酶同工酶(CK-OH)、乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)、Mb水平高(P<0.05) ,较T2DM组CK-OH、LDH高(P<0.01)。多元逐步回归分析显示,HbA1c是影响CK、Mb的主要因素(r2分别为0.052、0.051,P<0.01),FPG是影响LDH的主要因素(r2为0.043,P<0.01),BMI、HbA1c是影响CK-OH的主要因素(r2分别为0.025、0.040,P<0.05)。 结论 酮症倾向T2DM患者肌酶谱和Mb水平存在异常变化。

  【Abstract】 Objective To explore the changes of myocardial enzyme and myoglobin in patients with newly diagnosed ketosis-prone T2DM. Methods 97 cases with newly diagnosed ketosis-prone T2DM, 49 cases newly diagnosed nonketotic T2DM and 48 cases normal control (NC) people were collected. The levels of myocardial enzymes and myoglobin were tested in three groups, analyzing the related factor influencing the change of serum myocardial enzymes and myoglobin. Results The levels of creatine kinase, creatine kinase isoenzyme, lactate dehydrogenase, and myoglobin were significantly higher in the patients with newly diagnosed ketosis-prone T2DM than in NC group (P<0.05) . The levels of creatine kinase isoenzyme, lactate dehydrogenase and myoglobin were significantly higher in the patients with newly diagnosed ketosis-prone T2DM than in the patients with newly diagnosed nonketotic T2DM (P<0.01). Multiple stepwise regression analysis showed that HbA1c was the impact factor influencing CK and Mb (r2 is 0.052, 0.051, P<0.01), FPG was the impact factor influencing LDH (r2 is 0.043, P<0.01), BMI and HbA1c were the impact factor influencing CK-OH (r2 is 0.025, 0.040, P<0.05). Conclusion There was changes of serum myocardial enzymes and myoglobin in the process of newly diagnosed ketosis-prone T2DM.
