【摘要】目的 评价持续皮下胰岛素注射(CSII)治疗在胰岛素脱敏治疗中的应用价值。方法 对7例胰岛素过敏患者采用胰岛素持续皮下泵入法进行脱敏治疗,以0.05 U/h稀释胰岛素溶液为起始基础率,逐步增加输注速度和浓度至临床所需剂量,在此基础上调整治疗方案为餐前皮下注射胰岛素联合口服药物治疗。结果 7例患者均成功脱敏,改为皮下注射后也未再出现过敏症状。与治疗前相比,随访3个月后患者的HbA1c、FPG、2hPG均明显下降(P<0.01)。结论 胰岛素泵能较好地模拟生理性胰岛素分泌,可更好的应用于胰岛素脱敏治疗。

【Abstract】Objective To evaluate the application value and nursing methods of insulin pump in desensitization therapy of insulin. Methods 7 patients allergic of insulin received desensitization therapy by continuous subcutaneous insulin infusion.The diluted insulin solution was pumped in initial basal rate of 0.05 U/h,and the basal rate and insulin concentration increased gradually until the insulin dosage reached clinical goal.After that,continuous subcutaneous insulin infusion was replaced by regimen of insulin subcutaneous injection plus oral hypoglycemic agents.Results In all of 7 cases marked success, the allergy symptoms also did not appear again after subcutaneous injection.Compared with before therapy,the HbA1c(%)、FPG、2hPG were significantly decreased after follow up 3 months(P<0.01). conclusion Insulin pump can better to simulate the secretion of physiologic insulin, so the application of insulin pump is a better therapeutic method in insulin desensitization.
