【摘要】 目的 了解糖尿病足溃疡感染患者的病原菌分布与药敏状况。 方法 对收治的57例糖尿病足溃疡感染患者分泌物进行病原菌培养和药敏结果分析。 结果 57例患者分泌物标本中43例患者(75.4%)培养出病原菌69株,其中革兰阳性菌37株,革兰阴性菌29株,真菌3株;单一菌感染42例,混合菌感染15例,药敏试验结果提示多药交叉耐药,革兰阳性菌对万古霉素、氯霉素及头孢菌素较敏感;革兰阴性菌对亚胺培南及加β-内酰胺酶抑制剂的抗菌药物较为敏感。 结论 糖尿病足溃疡感染的病原菌分布广泛,对抗菌药物耐药率高,早期联合选择敏感抗菌药物治疗是糖尿病足溃疡感染治疗的关键。

【Abstract】 Objective To study the distribution of pathogens and their sensitivity to antibacterial drugs in diabetic foot ulcer. Methods Totally 57 patients with diabetic foot ulcer were enrolled and the exudate from the ulcer was collected for culture and antimicrobial sensitivity test. Results From 43 (75.4%) of 57 patients, 69 strains of pathogenic bacteria were isolated, of which, 37 were Gram-positive bacteria, 29 were Gram-negative bacteria, and 3 were fungi. Forty-two cases were single microbial infection and 15 cases polymicrobial infection. According to sensitivity test, multi-drug resistance was found. Gram-positive bacteria were sensitive to vancomycin, chloramphenicol, and cephalosporin, while Gram-negative bacteria were sensitive to imipenem and antibacterial drugs with β-lactamase’s inhibitor. Conclusion An extensive distribution of pathogenic bacteria in diabetic foot ulcer infection exists and some are multi-drug resistant. A combination of sensitive antibacterial drugs should be applied as early as possible is the key in treatment.
