【摘要】 目的 观察胰岛β细胞分泌功能与胰岛素抵抗指数(HOMA-IR)在T2DM中的变化。 方法 T2DM患者268例,行75 g OGTT或100 g馒头餐试验,以及C-P释放试验,比较组间各项相关指标的差异。 结果 与病程<5年组比较,病程>10年组胰岛β细胞功能(HOMA-β)下降(P<0.05)。病程>10年组随FPG增加,HOMA-β下降(P<0.01),HOMA-IR增加,但差异无统计学意义(P>0.05)。 结论 长期良好的控制血糖,可延缓胰岛β细胞功能的衰竭。病程对HOMA-β的损伤低于高血糖。

【Abstract】 Objective To evaluate β cell function,insulin resistance and their changes in patients with type 2 diabetes mellitus (T2DM). Methods A total of 268 T2DM patients were enrolled in this study. 75 g OGTT or 100 g steamed bread meal test and C-peptide release test were measured in all the subjects. Results The β cell function index (HOMA-β) decreased significantly during the progress of T2DM (P<0.05). In T2DM duration>10 years group,the decrease of HOMA-β and the increase in HOMA-IR associated with the increase of FPG did not reach the significant difference (P>0.05). Together with the raising of FPG level,HOMA-β decreased,and the HOMA-IR increased significantly (P<0.01). Conclusion Long-term,well-controlled blood glucose could delay the exhaustion of β cell function. Injury to β cell function based on duration of diabetes is less severe than that of hyperglycemia.
