糖尿病周围神经病变患者颈动脉内-中膜厚度、颈动脉僵硬度与糖化血红蛋白的相关性研究

  【摘要】 目的 探讨DPN患者颈动脉内-中膜厚度(CIMT)、颈动脉僵硬度与HbA1c的关系。 方法 选取T2DM组60例、DPN组100例及健康体检者(NC)60名,测定HbA1c水平、CIMT和颈动脉僵硬度。 结果 DPN组HbA1c[(8.62±2.71)% vs (4.20±0.47)%,(7.59±1.98)%],CIMT[(1.01±0.11) vs (0.70±0.07),(0.81±0.09)mm]和颈动脉硬度[(827.6±123.7)vs (521.2±89.3),(629.3±113.5)]高于NC、T2DM组(P<0.05);DPN组病程高于T2DM组(P<0.05);DPN组HbA1c≥10.0%亚组CIMT[(1.11±0.09) vs (0.94±0.07),(0.73±0.06)mm]和颈动脉僵硬度[(901.5±241.5)vs(724.5±159.9),(574.1±145.3)]高于8.0%≤HbA1c<10.0%亚组和7.0%≤HbA1c<8.0%亚组(P<0.05);相关分析结果显示,HbA1c与CIMT、颈动脉僵硬度呈正相关(r=0.107、0.213,P<0.05)。 结论 DPN患者CIMT、颈动脉僵硬度与HbA1c呈正相关,且HbA1c是CIMT和颈动脉僵硬度的危险因素。

  【Abstract】 Objective To explore the relationship between HbA1c and carotid artery intima-media thickness (CIMT),carotid arterial stiffness in diabetic patients with peripheral neuropathy. Methods A total of 220 subjects were enrolled in this study and divided into three groups:T2DM group (n=60),DPN group (n=100),and healthy individuals as NC group (n=60). Serum HbA1c,CIMT and carotid arterial stiffness were measured. Results HbA1c [(8.62±2.71)% ],CIMT [(1.01±0.11)mm] and carotid arterial stiffness [(827.6±123.7)]was significantly higher in DPN group than in NC group [(4.20±0.47) %,(0.70±0.07) mm,(521.2±89.3)] and T2DM group [(7.95±1.98) %,(0.81±0.09) mm,(629.3±113.5)] respectively(P<0.05).The duration was significantly longer in DPN group than in T2DM group(P<0.05). CIMT [(1.11 ± 0.09)mm] and arotid arterial stiffness [(901.5 ± 241.5)] was higher in patients with HbA1c≥10.0% than in patients with HbA1c between 8.0%~10.0% [(0.94±0.07)mm,(724.5±159.9)] and patients with HbA1c between 7.0%~8.0% [(0.73±0.06)mm,(574.1±145.3)] respectively(P<0.05).Association analysis showed that HbA1c had a positive correlation with CIMT and carotid arterial stiffness (r=0.107,0.213,P<0.05).ConclusionCIMT and carotid stiffness are positively correlated with HbA1c in DPN patients.HbA1c is a risk factor for CIMT and carotid stiffness.
